What is the offset of the SCSI command byte?

I would like to use a filter based monitor to count SCSI reserve commands (0x16)

I looked at a trace that I had and it looks like the CDB starts at offset 40 in all the scsi commands that I looked at (mostly inquiry and test unit ready). Also just to be safe would you want to also count 0x56 (reserve 10) too, so the command would be (modifying your example):perfAddUserMonitor 2, "40, 0xff, 0x16, 0x56", "Reservations"
