Brocade 81B - Heart Beat of IOC 1 has failed

Hi,
I have w2k8 r2 server with HP StorageWorks (Brocade) 81B PCI-e FC HBA Single Port 8Gb Fibre Channel Host Bus Adapter. Server is directly connected to storage array. After rebooting the machine my HBA seems to be crashed. In HCM log I have following errors:

331    CRITICAL    10:00:00:05:1E:FA:B6:C8    IOC    Crash    Heart Beat of IOC 1 has failed.    mar 14 2012  08:28:30  PM. It means that the adapter's firmware is not responding. I would suggest open a support case with HP first and then with Brocade support. Gather all infomation such as server vendor (In this case is a DL xxx model?), gather the supportsave log file if available (Example: If runs Windows-->Launch Host Connectivity Manager --> Tool --> Supportsave) and for Unix OS--> just type bfa_supportsave. Gather the Brocade HP branded 81B serial number first before open a ticket with HP and Brocade. Also, gather the switch's supportsave (If Brocade) and any relevant info related for troubleshooting.

By the way, have you re-seat the SFP+ on the 81B adapter? Was it a Brocade branded and/or HP branded SFP+ optic?
